Below is our Attendance Form for our 2006-07 Dual  Meets. You only need to fill out this form if there is a meet on the calendar you cannot attend.

Please fill out the following Required Information for the form, then proceed:

*Swimmer First Name  
*Swimmer Last Name  
*Your E-mail Address  

Please Select the Meets and Days where applicable that you will and will not be attending on the form below - you must select an answer for all meets. If you choose "Undecided" we will automatically enter you into the Meet unless you notify Dan Warner nssc1@aol.com  by the Deadline of the Meet Entry.
